About The Position

HopeHealth is seeking an experienced Palliative Nurse Practitioner (NP) for its affiliate HopeHealth Hospice & Palliative Care, primarily at St. Annes Hospital. The Palliative Care Nurse Practitioner collaborates with referring providers and consultants to provide relief of suffering for patients and families facing serious illness. Responsibilities include assessing and recommending treatments for symptoms such as pain, dyspnea, anxiety, and fatigue, as well as compassionately communicating serious news and helping patients and families establish clear care goals. This role requires a highly collaborative individual who serves as an expert in hospice and palliative care.

Requirements

Graduated from an accredited master’s degree program in Nursing and/or Clinical Doctorate program in nursing.
Certified as a Family/Geriatric or Adult Nurse Practitioner.
Certified as a Registered Nurse Practitioner in the state of RI required.
Minimum of one to three years' experience as a nurse practitioner or physician's assistant, preferably in Palliative Care.
Minimum of 2 years' experience in geriatrics, acute care, hospice, or oncology.
Current RI and MA licenses as RN and Registered Nurse Practitioner or Physician's Assistant.
Current RI and MA licenses for prescriptive authority, RI and MA controlled substance license, DEA license.
Valid driver's license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.

Nice To Haves

Experience with pain and symptom management is highly preferred.
Knowledge of third-party reimbursement procedures and requirements is helpful.
Experience in hospice and palliative care is highly preferred and with certification as advance practice in hospice and palliative care.
Experience in primary care within a hospital or nursing home preferred.

Responsibilities

Provide direct patient care, including new and follow-up consults for seriously ill adults in various settings (homes, assisted livings, nursing facilities, hospitals, clinics).
Complete pain assessments and make pain management recommendations.
Assess and recommend management for other non-pain symptoms like nausea, fatigue, dyspnea, and anxiety.
Assess patient's disease trajectory and co-morbidities, and in conjunction with treating physicians, formulate prognosis.
Serve as a consultant, fostering positive and collaborative relationships with referring medical providers.
Make recommendations regarding further treatment and end-of-life options based on prognosis and patient/family goals.
Provide counseling and coordinate care with patients, families, providers, and facilities.
Improve clinical outcomes by conducting research or applying research findings.
Demonstrate knowledge of HHHPC policies, procedures, and pertinent State and Federal Rules and Regulations.
Provide accurate and appropriate documentation and timely submission of billing.
Consistently apply HHHPC values, complying with mission, policies, procedures, and objectives in quality assurance, safety, environment, infection control, and HIPAA compliance.
Serve as an expert in hospice and palliative medicine, providing informal and formal education, and serving as a preceptor to students or new hires.
