About The Position

Notre Dame Health Care is seeking a full-time Palliative Care Nurse Practitioner for Notre Dame At Home. The Nurse Practitioner will collaborate with the Medical Director, hospice physician, patient physicians, and team members to deliver and support appropriate clinical care under Hospice Federal and State conditions of participation. Notre Dame Health Care, rooted in the heritage of the Sisters of Notre Dame de Namur, is committed to the physical, spiritual, and emotional care of its residents and patients. As one of Worcester’s largest healthcare organizations, NDHC provides a range of services including long term/skilled rehab care, residential assisted living care, adult/pediatric Hospice and palliative care, and education. The Palliative team is looking for a full-time Nurse Practitioner.

Requirements

Graduate of an accredited school of nursing
Graduate of an accredited Nurse Practitioner Program
Current Advanced Practice Registered Nurse Licensure in the State of Massachusetts
Current Nurse Practitioner certification with primary specialty Hospice and Palliative Care preferred
1 year of previous experience in Hospice and Palliative Care is preferred
Demonstrates leadership capabilities with the ability to relate well to staff and other professionals
Must work well in a team environment
Must support and promote a kind and respectful workplace
Must possess strong organization skills
Must maintain a valid driver’s license, dependable transportation, and proof of current automobile insurance
Expected to participate in appropriate continuing education as required to maintain licensure
Possesses excellent verbal and written communication skills
Possess excellent time management skills
Possess strong computer skills, including Word, Excel, and Power Point

Nice To Haves

Current Nurse Practitioner certification with primary specialty Hospice and Palliative Care preferred
1 year of previous experience in Hospice and Palliative Care is preferred

Responsibilities

Interact with Management team on a professional level to ensure administrative needs are being met.
Functions as an integral part of an interdisciplinary team in order to enhance the mission, policy, and philosophy of Notre Dame Hospice and Palliative Care Services.
Responsible for the planning, coordinating, providing and documenting care for terminally ill patients and their family members.
Establish home health aide plan of care as well as indirectly and directly supervising the plan of care per regulations.
Manages cost and quality of care as well as all medications, DME, services, and medical supplies.
Document accurate and ongoing assessment of patient status via a variety of mediums of communication (verbal, written, email, computer documents and databases). Document patient care reflecting nursing interventions, patient response to care, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record.
Maintain up-to-date charts and records on patient care and regular communication with the patient’s physician regarding changes in the patient’s plan of care.
Supervise and maintain ongoing effective communication with other hospice personnel involved with patient care. This may involve formal and informal team meetings in addition to IDT.
Works with the Admission Team to support access to Hospice in a timely manner.
