About The Position
Impact Health PC is seeking a PRN Palliative Care Nurse Practitioner to provide compassionate, patient-centered care to residents in skilled nursing facilities in Christiana, Pennsylvania. The role involves assessing and managing symptoms related to serious and complex illnesses, developing individualized treatment plans, supporting goals-of-care discussions, and collaborating with physicians, facility staff, patients, families, and interdisciplinary care teams. The position offers flexible scheduling based on the practitioner’s availability and facility needs, clinical autonomy within scope of practice, and support from an experienced clinical team.
Requirements
- Active Nurse Practitioner (NP) license in Pennsylvania.
- Strong clinical assessment, communication, and interpersonal skills.
- Comfort managing symptoms and supporting goals-of-care discussions with patients and families.
- Ability to work independently within scope of practice and collaboratively with an interdisciplinary care team.
- Proficiency with EHRs and clinical technology.
Nice To Haves
- A minimum of 2 years of post-certification clinical experience is preferred.
- Experience in palliative care, hospice, skilled nursing facilities, hospitalist medicine, emergency departments, or intensive care is preferred.
Responsibilities
- Provide compassionate, person-centered palliative care using evidence-based practices.
- Conduct comprehensive patient assessments and develop individualized treatment plans.
- Manage symptoms and provide ongoing follow-up for patients with serious and complex illnesses.
- Support goals-of-care and advance care planning discussions with patients and families.
- Collaborate with physicians, nursing staff, specialists, and interdisciplinary care teams.
- Educate patients, families, and facility staff regarding palliative care and treatment options.
- Complete timely and accurate clinical documentation.
- Maintain compliance with facility, state, and federal regulations.
