About The Position
Texas Health Physicians Group has an opening for a Full-Time Palliative Care Nurse Practitioner in Fort Worth, TX. This is a new opportunity to join a passionate team to provide comprehensive palliative care services to any patient and family dealing with a serious illness. This role focuses on the care and transition of patients in the inpatient setting and requires knowledge of hospice regulations, end-of-life care services, and advance care planning. The position utilizes Electronic Medical Records - EPIC.
Requirements
- Graduate of accredited Nurse Practitioner program.
- NP Certification and TX license required.
- At least 1-2 years Palliative Care experience required.
- Current RN license for the State of Texas.
- Certified Advanced Nurse Practitioner in the State of Texas.
- Excellent communication skills, with both co-workers and patients and families.

Responsibilities
- Provide comprehensive palliative care services to patients and families dealing with serious illness.
- Focus on the care and transition of patients in the inpatient setting.
- Utilize Electronic Medical Records - EPIC.

- Perform duties and responsibilities applicable to the THPG patient population and assigned setting.
