About The Position

As a Community-Based Palliative Care Nurse Practitioner, you will join a compassionate, mission-driven team dedicated to improving the quality of life for patients living with advanced or chronic illnesses. The Community-Based Palliative Care Nurse Practitioner (NP) provides advanced clinical support to patients with serious or life-limiting illnesses across home and facility settings. Working in close collaboration with interdisciplinary team members, the NP performs in-home evaluations, manages complex symptoms, facilitates goals-of-care discussions, and connects patients and families with supportive resources. This role emphasizes whole-person care, helping patients live as fully and comfortably as possible in the setting of their choice.

Requirements

  • Active, unrestricted Wisconsin Nurse Practitioner license with prescriptive authority (APNP).
  • Certification from an approved certifying body for advanced practice nursing (e.g., ANCC, AANP).
  • Current DEA registration.
  • Master’s degree in Nursing, with a minimum of one (1) year of experience as a Nurse Practitioner and one (1) year of experience in hospice, palliative care, or geriatrics.
  • Strong knowledge of Wisconsin hospice regulations and CMS guidelines.
  • Demonstrated excellence in leadership, organization, and communication.
  • Valid Wisconsin driver’s license, reliable transportation, and proof of auto insurance.
  • Ability to travel to patients’ homes and attend meetings at various locations.
  • Ability to maintain healthy boundaries and positive professional relationships.
  • Self-directed with the capacity to manage stressful or emergency situations effectively.
  • Culturally sensitive with the ability to build rapport across diverse backgrounds.
  • Mature, poised under pressure, and able to manage emergencies independently.

Responsibilities

  • Deliver in-home and facility-based care to patients with serious illness, performing comprehensive assessments and managing physical, emotional, and psychosocial symptoms.
  • Engage in goals-of-care conversations and facilitate advance care planning with patients, families, and caregivers.
  • Collaborate with referring providers, care managers, and community resources to coordinate services and ensure continuity of care.
  • Order and interpret laboratory and diagnostic tests, diagnose health conditions, and document findings in the patient’s medical record.
  • Document thoroughly and promptly in the EMR; maintain effective, compassionate communication with patients, families, and the care team.
  • Participate in interdisciplinary team meetings to align care plans and support transitions between palliative, hospice, and curative services.
  • Provide clinical education and emotional support to patients and caregivers to empower informed decision-making.
  • Contribute to program development and quality improvement efforts, helping to expand and enhance community-based palliative services.
  • As needed, conduct timely, CMS-compliant face-to-face assessments to evaluate continued hospice eligibility.
  • Provide clinical information to the hospice medical director to support eligibility decisions for hospice recertification.
  • Comply with all HIPAA requirements in accordance with federal, state, and organizational policies.
  • Attend trainings and staff meetings as required.
