About The Position

Emmanuel Hospice is launching a new palliative care program and is seeking a Palliative Care Manager to be a cornerstone in its development. This role involves shaping the program's vision, structure, and impact, guiding the creation of innovative care models to support patients with serious illness and their families. The Palliative Care Manager will ensure the program operates with excellence and regulatory compliance, aligning with the mission to support patients and families. This full-time, salaried position is ideal for a social worker who thrives in diverse, collaborative, and mission-driven environments.

Requirements

  • Master's degree in social work required
  • Experience in Electronic Medical Records and Microsoft Office products is required
  • Strong interpersonal, communication, and leadership skills with a demonstrated ability to collaborate effectively across disciplines, and guide teams through periods of growth and change.

Nice To Haves

  • Hospice or palliative care related leadership experience preferred
  • Prior program development experience preferred

Responsibilities

  • Lead all program startup activities, including workflows, policies, and operational design
  • Manage program budget, financial performance, billing processes, and revenue cycle
  • Supervise and support palliative care staff in collaboration with the Medical Director
  • Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
  • Support provider credentialing and payer contracting
  • Ensure efficient scheduling, referrals, documentation, and care transitions
  • Drive program growth, customer service, and performance improvement
  • Deliver social work services during start up and as needed
  • Participate in the Manager On-Call Rotation for hospice services
  • Travel within the service area to patient homes and healthcare facilities
