About The Position

This role is for a social worker who is ready to lead and shape a meaningful palliative care program. The Palliative Care Manager will be instrumental in launching and growing a brand-new palliative care program from the ground up, working alongside a mission-driven team that values courage, compassion, and creativity. The manager will guide the creation of innovative care models to support patients facing serious illness and their families, ensuring the program becomes an essential part of Emmanuel Hospice's continuum of care. The position reports directly to the CEO and is responsible for ensuring the program upholds excellence, regulatory compliance, and the core mission of supporting patients and families.

Requirements

  • Master's degree in social work required
  • Experience in Electronic Medical Records and Microsoft Office products is required
  • Strong interpersonal, communication, and leadership skills with a demonstrated ability to collaborate effectively across disciplines, and guide teams through periods of growth and change.

Nice To Haves

  • hospice or palliative care related leadership experience preferred
  • Prior program development experience preferred

Responsibilities

  • Lead all program startup activities, including workflows, policies, and operational design
  • Manage program budget, financial performance
  • Supervise and support palliative care staff in collaboration with the Medical Director
  • Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
  • Support provider credentialing and payer contracting
  • Ensure efficient scheduling, referrals, documentation, and care transitions
  • Drive program growth, customer service, and performance improvement
  • Deliver social work services during start up and as needed
  • Participate in the Manager On-Call Rotation for hospice services
