About The Position

Palliative Care APRN wanted to join a diverse IDT that includes Physicians, chaplaincy, social work, RN, and pharmacy support. This diverse practice can be tailored to your clinical interests including an inpatient palliative medicine consulting service, outpatient palliative clinic, 16 bed inpatient hospice unit, and homebased hospice visits. Shared call that is conducive to quality work-life balance.

Requirements

  • Demonstrated skills and knowledge as a Nurse Practitioner in area of expertise
  • Excellent observation, problem solving skills, mathematical skills, and nursing skills.
  • Knowledge of community, professional and education resources.
  • Excellent communication skills written and verbal amongst professionals.

Nice To Haves

  • One (1) year nurse practitioner experience preferred.
  • One (1) year hospice or palliative nursing experience preferred.

Responsibilities

  • Reviews clinical information and completes new consultations and follow ups in a timely manner working with the interdisciplinary team (physicians, social workers, and chaplains)
  • Acts as an integral part of the treatment team for patients with cancer-related illness and life limiting chronic illnesses to assisting in determining goals of care.
  • Acts as an integral part of the treatment team for cancer-related patients to manage symptoms via various routes in collaboration with palliative care physician.
  • Provides extensive education regarding chronic diseases and how they impact quality of life.
  • Communicates with treatment team including other providers on changes to plan of care.
  • On call coverage required that includes cross coverage for palliative care clinic and hospice inpatient and home care team
