About The Position

Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work-life balance flexibility (no on call or weekends)! We are seeking a full-time (32-40 hours per week) palliative care Nurse Practitioner for float between our centers located in the eastern West Virginia panhandle (cities include Charles Town, Martinsburg and Shepherdstown). Requirements

  • Graduate from an accredited School of Nursing program.
  • Licensed as a registered nurse.
  • Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing.
  • Concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need.
  • Minimum of two years experience working with the adult or geriatric population is preferred.
  • In states where practice allows; APRN with an unrestricted professional license and DEA, for each state in which they practice.

Responsibilities

  • Provide primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center.
  • Provide palliative care services to nursing center residents with serious illness in collaboration with the primary care team, with emphasis on pain and symptom management, psychosocial support, and advance care planning.
  • Serve as a specialized consultant within the interprofessional team, providing recommendations for the plan of care while incorporating patient goals and preferences.
  • Provide guidance and education to the patient, family, primary care team, nursing, social work and rehabilitation therapies regarding risks and benefits of relevant interventions to facilitate shared-decision making.
  • Utilize evidence-based practice and standards of care for managing patient care, including assessment, diagnosis, interpreting labs and diagnostics, development and implementation of treatment plans, and advanced care planning.
  • Follow-up and evaluate patient status as necessary and complete all necessary documentation.
  • Provide health education to patients and families.
  • Demonstrate continued professional competence through continuing education and maintenance of certification.
  • Support the development of nursing staff in collaboration with the Center Nursing Executive programs.
  • Participate in center-based quality initiatives, including avoiding hospital transfer and ACO initiatives.
  • Ensure patients and families receive high-quality service in a caring and compassionate atmosphere.
  • Adhere to Coding and Billing Compliance Program, including timely submission of billable charges.
  • Comply with the AlignMed Partners Time Off and On Call policies.
  • Perform other duties as requested.
  • Implement and support AlignMed Partners clinical initiatives.
