About The Position

Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations. Under the general supervision of the Medical Director, & Manager Health Services, the incumbent participates as a member of the multidisciplinary team in the care of complex patients. Assesses, plans, implements, coordinates, and evaluates nursing care for an assigned population of patients and families. Applies advanced practice, knowledge and principles to the provision of nursing care. Receives and responds to referrals from inpatient units, ambulatory care clinics, and the community for teaching, consultation, assessment and follow-up. Maintains a commitment to education that ensures standardization and dissemination of current evidence-based information.

Requirements

  • Minimum of 4 years recent related clinical experience in nursing required.
  • Baccalaureate Degree in Nursing required.
  • Current registration or eligibility for registration with the College of Registered Nurses of Manitoba (CRNM).
  • Excellent interpersonal, verbal and written communication skills.
  • Ability to function in a computerized environment.
  • Ability to problem solve in complex situations.
  • Demonstrated strong critical thinking skills.
  • Knowledge of adult learning and teaching strategies.
  • Demonstrated ability to cope with change.
  • Commitment to ongoing professional development.
  • Ability to perform the essential physical requirements of the position.
  • Minimum to moderate physical effort required.
  • A current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.

Nice To Haves

  • Master’s Degree in Nursing is preferred (equivalent combination of education and experience may be considered).
