About The Position

Lighthouse Behavioral Wellness Centers is seeking a PACT Nurse to guide and empower individuals dealing with mental illness, trauma, or addiction. This role involves advocating for and connecting consumers with essential behavioral health, wellness, primary, and specialty care services. The PACT Nurse is responsible for leading the development and ongoing management of comprehensive, individualized care plans that help consumers and their families understand their clinical conditions, manage symptoms, navigate treatment options, and address non-clinical factors impacting their quality of life and outcomes. The position requires meeting performance standards and contributing to the team's success, with Lighthouse providing necessary tools and training.

Requirements

• Completion of the curriculum requirements for an Associate’s degree in nursing, possession of a valid, current, Oklahoma License as a RN as approved by the Oklahoma Board of Nurse Registrations and Nursing Education to practice professional nursing OR completion of a Licensed Practical Nursing program and a valid, current, Oklahoma License as an LPN by the Oklahoma Board of Nurse Registration and Nursing Education
• 2 years of experience as an LPN (if applicable)
• Demonstrated knowledge of the integrated care philosophy.
• Demonstrated skill in working with individuals with Serious Mental Illness.
• Demonstrated skill in treating social and emotional problems.
• Demonstrated skill in following oral and written instructions and implementing care pathways.
• Demonstrated skill in exercising good judgment.
• Core competencies in relation to individuals with whole health wellness, co-occurring disorders, trauma informed care and collaborative care.
• Ability to use clinical tools, data and other information to complete assessments of consumer needs.
• Knowledge of and ability to clearly explain the reason for prescribed treatments and treatment plans to patients.
• Knowledge of and ability to follow case and quality management processes in compliance with regulatory guidelines and company procedures.
• Knowledge of the principles and practices of nursing.
• Knowledge of current social and economic problems.
• Knowledge of principles of family economics and budgeting.
• Knowledge of human relations.
• Knowledge of community, state and national health and welfare resources.
• Knowledge of social assistance requirements.

• Must complete Lighthouse required training and resulting follow up and consultation as required at hire, annually and as directed by Supervisor.

Responsibilities

• Lead the development and ongoing management of a comprehensive, individualized care plan with each consumer.
• Empower, uplift, and help others heal from mental illness, trauma, or addiction.
• Advocate and link consumers with complex illnesses to needed behavioral health and wellness services, as well as primary and specialty care.
• Assess consumer needs using clinical tools, data, and other information.
• Provide collaboration and input into the creation of individualized and comprehensive care plans.
• Clearly explain the reason for prescribed treatments and treatment plans to patients.
• Communicate with patients, physicians, and insurance companies to secure successful outcomes.
• Follow case and quality management processes in compliance with regulatory guidelines and company procedures.
• Collaborate with others to identify personal strengths and goals within the Eight Dimensions of Wellness.
• Work within an integrated team (inpatient and outpatient) to facilitate care by assessing consumer needs, evaluating treatment options, creating care plans, addressing treatment barriers, coordinating care with internal and external providers, and monitoring progress.
• Provide services within the community or consumer homes to promote engagement and assist in stabilization.
• Provide varying types of individualized care management, nursing and wellness services in various settings to individuals, families and groups to promote emotional or psychological change to alleviate mental disorders and facilitate recovery.
• Monitor and evaluate the needs and abilities of the consumer and assist them to function more adequately through use of their own strengths and through use of community resources.
• Complete all agency documentation of services in a thorough, accurate and timely manner.
• Assist clients to adjust and adapt themselves to their environment, care and treatment.
• Assist in the development and implementation of Care Pathways.
• Serve as a role model for wellness behaviors.
• Apply the principles and processes of coaching when collaborating with others.
• Provide referrals to community resources for housing, employment, medical, nutrition education, weight management, Oklahoma Tobacco Helpline, and other community or wellness-related services and resources.
• Create partnerships within the local community to enhance consumer access to resources that support the individualized Care Plan goals.
• Perform all job-related travel normally associated with this position.
• Prepare for and actively participate in huddles, Care Planning and staff meetings.
• Complete Lighthouse required training and resulting follow up and consultation as required at hire, annually and as directed by Supervisor.
• Perform other reasonably related duties as assigned by the immediate supervisor or other management as requested.
