About The Position

L’esprit is seeking a compassionate, motivated, and community-focused PACT Generalist to join our multidisciplinary Program of Assertive Community Treatment (PACT) team. This position provides intensive, community-based mental health support to adults living with Severe and Disabling Mental Illness (SDMI). PACT is an intensive community-based treatment program that consists of an interdisciplinary team who provides 24/7 community-based care to adults with severe, disabling mental illness. The PACT model utilizes an integrated, multidisciplinary team of providers who deliver services directly to individuals in their homes and within communities; meeting the clients where they are. The program aims to promote independent living while keeping clients out of hospitals and crisis centers. L'esprit's PACT program operates in Park, Gallatin, and Sweet Grass counties.

Requirements

Strong interpersonal and communication skills
Ability to build positive relationships with clients and team members
High school diploma or equivalent required
Mental health experience strongly preferred
Valid Montana driver license and reliable transportation with a clean driving record

Responsibilities

Provide person-centered client engagement and support services in community settings
Assist clients with daily living skills, treatment planning, and recovery goals
Support clients in attending medical, psychiatric, employment, and community appointments
Help clients navigate housing, benefits, and community resources
Coordinate care with family members, service providers, and community partners
Participate in treatment team meetings and collaborative care planning
Complete accurate and timely clinical documentation
