About the RoleJob Summary: The PACE Registered Nurse provides comprehensive clinical care, care coordination, field-based services, and intake assessments for PACE participants and prospective enrollees. This role functions as an integrated member of the Interdisciplinary Team (IDT), supporting participant health outcomes through assessment, care planning, coordination, education, and hands-on nursing services across clinic, home, and community settings. The RN ensures compliance with federal, state, and CMS PACE regulations while delivering participant-centered, high-quality nursing care throughout the participant lifecycle—from intake through ongoing enrollment.

Requirements

  • Strong clinical assessment, judgment, and documentation skills.
  • Knowledge of federal, state, and CMS PACE regulations.
  • Ability to function independently in clinic and field environments.
  • Excellent verbal and written communication skills.
  • Strong organizational and time-management abilities.
  • High level of professionalism, integrity, and confidentiality.
  • Proficiency with EMR and clinical software systems.
  • Ability to operate effectively in a fast-paced, changing environment.
  • Customer-service orientation with participants and families.
  • Current Registered Nurse (RN) license in good standing (state-specific).
  • Current CPR Certification.
  • Valid Driver’s License (for field-based work).

Nice To Haves

  • ACLS certification preferred (as applicable).
  • Experience in geriatric, home health, or PACE preferred.
  • Prior experience with in-home assessments and care coordination strongly preferred.
