About The Position

POSITION SUMMARY: The PACE Registered Nurse (RN) demonstrates the knowledge and skills necessary to provide participant care that is appropriate to the ages of the participants served. Coordinates inter-disciplinary (IDT) care to participants, which may include nursing, therapy, social work, home care aide, adult day, or any other services as needed. Case manages with internal and external disciplines as needed. Provides participant-centered care, in the clinic, day center and in the home, using critical thinking skills to ensure positive participant outcomes. The RN is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management and episode management as they are serving in an RN Case Manager role.

Requirements

  • High School Diploma/ GED
  • Illinois Reg Nurse License
  • Current American Heart Association HealthCare Provider BLS is required prior to start date of employment
  • Experience as an RN in a healthcare environment
  • A minimum of one years' experience working with the frail or elderly population or, if the individual has less than one year of experience but meets all other requirements must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Excellent interpersonal and communication skills
  • Solid computer skills, including proficiency with Microsoft software
  • Strong analytical and problem-solving skills, with the ability to be detail oriented
  • Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty
  • Maintains a valid driver's license

  • Bachelor’s Degree in Nursing
  • Electronic Medical Record and Microsoft application basic proficiency
