The PACE Clinic RN has the primary responsibility of overseeing clinical acuity within the day center. This position will spearhead any clinic-based triage that is needed in real-time based on clinical presentation and symptomology. This position will function as a link in communication between participants and providers, judiciously establishing clinical criteria that is reported to physician and mid-level providers for appropriate and timely care management. This nurse will be in communication with other center-based staff as well as staff operating in the field, processing and relaying information regarding participant conditions and needs.

Requirements

  • Valid driver's license.
  • Current valid RN Licensure in good standing, received from a qualified, accredited school of nursing
  • BLS Certification required within 90 days of hire.
  • Minimum 1-year clinical experience working with a frail or elderly population.

Nice To Haves

  • Experience with clinical triage in an acute care or transitional care setting is preferred.

Responsibilities

  • Establish a functional workflow with all PACE center-based providers.
  • Receive communication from Center PCA’s, Center nurses, Homecare PCA’s, Field-based nurses, transportation, external providers and many other roles in the clinic, community, and homecare settings.
  • Medical triage of clinical presentations and reported symptomology among participants, in order to prioritize reporting to providers and provision of care.
  • Utilize services within the clinic appropriately across the affected population.
  • Leverage tools and resources at their disposal to maintain the highest standards of clinical quality and excellence in care.
  • Function as a bridge of communication between providers and all participant-facing caregivers, internal and external.
  • Assist with milieu management in the PACE Day Center
  • Be prepared to assist with PCA duties in the PACE Day Center, including matters of personal ADL assistance, dietary assistance, recreational assistance, and other needs as delegated by the Center Manager.
  • Operate as a member of the nursing team responsible for reviewing all incoming communications from participants, reviewing judiciously and responding appropriately.
  • Collaborate with the IDT, including pharmacy for medication needs and assist in maintaining dispensation timelines.
  • Collaborate with Therapy team and Center PCA’s to ensure functional needs are addressed, and changes to functional status are communicated.
  • Participate in Quality Improvement projects as assigned.
  • Additional center-based and field-based duties, as assigned by the Center Manager.
