As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: Monday- Friday 8:00am-4:30pm Position Summary: The Intake Registered Nurse (RN) is responsible for completing key components of the PACE intake and enrollment process. This role ensures that prospective participants receive timely, comprehensive nursing assessments and that the Interdisciplinary Team (IDT) is provided with accurate clinical information to support appropriate enrollment decisions. The Intake RN conducts home and clinic visits, evaluates clinical and functional needs, and supports a smooth, compliant transition into the PACE program.

Requirements

  • Must be a licensed driver
  • Graduate of an accredited school of registered nursing.
  • Currently licensed as Registered Nurse through the Kentucky Board of Nursing or a participating Compact State Board.
  • Minimum of one (1) year of nursing experience working with the frail and elderly.
  • Food handling permit required; to be completed upon hire.
  • Demonstrates strong nursing assessment skills, sound clinical judgement, and the ability to identify medical, functional, and psychosocial needs during intake and enrollment evaluations.
  • Skilled in communicating effectively with participants, families, caregivers, referral sources, and interdisciplinary team members; able to cope with family emotional stress and respect diverse lifestyles.
  • Demonstrates the ability to provide clear, timely, and accurate clinical documentation in the electronic health record
  • Possesses and maintains current CPR certification.
  • Have excellent observation skills, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner
  • Self-directed with the ability to work independently while effectively collaborating within an interdisciplinary team environment.
  • Flexible and cooperative in fulfilling role responsibilities and supporting program needs

Nice To Haves

  • Experience in home health, geriatrics, primary care, hospice, palliative care, preferred.
