About The Position
NeighborHealth is seeking a PACE Physician to provide comprehensive primary care to older adults with complex medical and functional needs enrolled in our nationally recognized Program of All-Inclusive Care for the Elderly (PACE). Physicians benefit from a predictable schedule, interdisciplinary team-based care, and the opportunity to deliver primary care services across PACE clinics, at home, assisted living, and nursing facilities. Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Massachusetts medical license or ability to obtain
Nice To Haves
- Geriatrics certification preferred but not required
- Experience caring for older adults or medically complex populations preferred
- Spanish language skills preferred
Responsibilities
- Provide primary care services to frail participants enrolled in the PACE program
- Care delivered across clinic, home, assisted living, and nursing facilities
- Care in all settings is provided in concert with an interdisciplinary care teams of health professionals in a progressive health care organization.
- Utilize EPIC electronic medical record
