About The Position

NeighborHealth is seeking a PACE Physician to provide comprehensive primary care to older adults with complex medical and functional needs enrolled in our nationally recognized Program of All-Inclusive Care for the Elderly (PACE). Physicians benefit from a predictable schedule, interdisciplinary team-based care, and the opportunity to deliver primary care services across PACE clinics, at home, assisted living, and nursing facilities. Requirements

  • Board Certified or Board Eligible in Internal Medicine or Family Medicine
  • Massachusetts medical license or ability to obtain

Nice To Haves

  • Geriatrics certification preferred but not required
  • Experience caring for older adults or medically complex populations preferred
  • Spanish language skills preferred

Responsibilities

  • Provide primary care services to frail participants enrolled in the PACE program
  • Care delivered across clinic, home, assisted living, and nursing facilities
  • Care in all settings is provided in concert with an interdisciplinary care teams of health professionals in a progressive health care organization.
  • Utilize EPIC electronic medical record
