The PACE Per Diem Physician is a progressive, population-based health role that provides exceptional quality participant care across the continuum. This role is essential in maintaining continuity of care during Primary Care Practitioner (PCP) absences, including vacation and sick leave. The clinician collaborates with the interdisciplinary team (IDT) to ensure comprehensive medical, social, and functional support for frail older adults, enabling them to live safely in the community. The PACE Per Diem Physician is responsible for taking after-hours call as needed, offers evaluation, treatment, coordination with specialists, health education, and executing Age-Friendly care for participants during scheduled shifts. This position requires excellent clinical skills across all aspects of medicine, exceptional collaboration capabilities, and proficiency in working with an interdisciplinary team (IDT). The PACE Per Diem Physician champions the PACE model of care and understands that medical decision-making is done in the context of overall goals of care.
