PACE Nurse Practitioner

Centra Health•Farmville, VA
•$110,000 - $142,250

About The Position

Centra’s PACE (Program of All-Inclusive Care for the Elderly) provides person-centered care to adults aged 55 and older in the setting most appropriate to each participant, including their home, a family home, long-term care, or assisted living. The capitated care model emphasizes meeting participants’ needs rather than productivity metrics. Care is coordinated by an interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff. Centra’s PACE program is seeking an Advanced Practice Provider to join its team and support an innovative, team-based care model. The role offers an opportunity to build meaningful relationships with older adults while working with a dedicated interdisciplinary team. The position is based in Farmville and serves a low volume of approximately five patients per day. The schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m., with no weekend or holiday work. Call is once every seven weeks from 5:00 p.m. to 8:00 a.m.; calls are triaged before reaching the provider, who is not expected to go out. Providers experienced in palliative care, hospice, primary care, or long-term care are encouraged to apply. Centra Medical Group is a network of nearly 600 employed providers within Centra Health, serving a population of 600,000 across Central and Southern Virginia. Centra also offers a transition-to-practice program through ThriveAP, providing new graduate NPs and PAs with a year of structured clinical education, mentorship, and peer learning.
