About The Position

The PACE Center Supervisor directly supervises staff members and daily PACE Center operations. This role involves providing supervision to departmental staff, coordinating all center activities, and ensuring efficient patient flow and customer service.

Requirements

  • Proficiency in written and verbal communication, basic mathematics, computer applications, and technical systems.
  • Minimum of two years of leadership experience or five years of progressively greater responsibility or significant contributions to projects and initiatives that demonstrate leadership skills.
  • Minimum credit score of 650.
  • Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  • Ability to use Microsoft Excel to analyze data, including formulas, functions, lookup tables, and other standard spreadsheet elements.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including embedded objects, transitions, and other elements.
  • Regularly required to sit and type.
  • Frequently required to stand or walk.
  • Occasionally squat, kneel, and reach above and below shoulder height to lift and move items up to 20 pounds.

Responsibilities

  • Provides supervision to departmental staff, including performance management, hiring/firing recommendations, and fostering a culture of accountability.
  • Ensures employees receive necessary training and on-the-job development.
  • Manages the work of the assigned team, assigning duties, providing guidance, and monitoring performance.
  • Ensures employees adhere to company workflows, procedures, and policies.
  • Recommends workflow and procedure changes based on observations.
  • Assists manager in ensuring department expenses stay within budget.
  • Maintains compliance with all employee-related reporting and tracking.
  • Coordinates all center activities, including care planning, interdisciplinary conferences, family conferences, and assessments.
  • Facilitates and leads morning meetings of the interdisciplinary team (IDT) and intake/assessment meetings.
  • Collaborates with the Manager to develop, implement, and track operational plans to improve health center performance goals.
  • Identifies opportunities and makes recommendations for improving customer service, patient throughput, and patient flow.
  • Coordinates with the enrollment team to hold “friendly visits” for new candidates.
  • Ensures patient information is collected, verified, and updated, and financial options are reviewed.
  • Ensures insurance eligibility verification is completed.
  • Understands and performs legal documentation and chart completion according to policy and procedures.
  • Completes periodic chart audits to ensure the PACE Center remains audit ready.
  • Conducts health center rounding to ensure customer service, team rapport, and compliance with OSHA, CMS, DHCS, accrediting bodies, regulatory agencies, and infection control guidelines.
  • Responsible for handling patient complaints and developing solutions.
  • Ensures all relevant logs are adequately maintained, including sterilization, medication, AIC, crash cart, OC for glucose, refrigerator, and oxygen tank logs.
  • Ensures supplies, medication, forms, and equipment are ordered and in stock.
  • Participates and assists with facilitating staff meetings.
  • Serves as a representative of FHCN PACE to the community.
  • Coordinates activities related to new participants.
  • Assists in facilitating efficient patient flow by coordinating the front and back office and ancillary services.
  • Utilizes dashboard and metric tools to ensure efficient patient flow and reduce wait times.
  • Ensures appointment schedules are maximized and spread throughout the shift.
  • Ensures staff follows operational workflows related to registration, eligibility, copay collections, vitals, referrals, and clinical procedures.
  • Prepares and posts staff schedules at least one month in advance.
  • Administers initial and annual skills/competency assessments.
  • Adheres to the Attendance and Absenteeism Policy.
  • Ability to present to and work at any FHCN location based on business need.
  • Performs other duties as assigned.
