About The Position

Under the direct supervision of the PACE Admissions RN Supervisor, the PACE Admissions RN is responsible for the development and timely submittal of the Level of Care (LOC) tool to the organizational shared drive and conduct initial In-Home Assessments (IHA) as part of the IHA Team. When needed/requested, conducts implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment.

Requirements

  • At least 1 year as an RN, in either home or community health, and within the frail or elderly population size.
  • Current and active RN license in the state of California
  • Current and active BLS certification.
  • Excellent interpersonal skills
  • Ability to read, write and comprehend medical information and terminology.
  • Ability to effectively present information, both verbal and written.
  • Demonstrated knowledge of the RN scope of practice.
  • Demonstrated ability to be culturally sensitive
  • Must be able to work effectively in a team environment.
  • Must be able to treat geriatric patients.
  • Only act within the scope of authority to practice
  • Competence in Microsoft Office Suite programs
  • Meet a standardized set of competencies by San Diego PACE and approved by CMS before working independently.
  • Physical clearance for reaching, bending, stooping, crouching, kneeling, and grasping; have full range of motion; ability to lift 50 lbs.; need to lift and transfer participants.
  • Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
  • Compliance with all mandated vaccinations and boosters is a term and condition of employment.

Nice To Haves

  • Experience in and/or knowledge of Adult Day Health Center or PACE.
  • Experience with strong critical thinking regarding chronic diseases.
  • Supervisory experience.
  • Bachelor of Science of Nursing (BSN)

Responsibilities

  • When needed, assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops initial plan of care.
  • Based on nursing assessment, completes, and submits LOC determination form to Admissions RN supervisor for review.
  • Reviews medical records as needed to complete LOC. If medical records can’t be accessed, complete LOC with other relevant information (i.e., hospital discharge information, medications, etc.).
  • Conducts initial Home/RN assessments at participant’s home during the team IHA.
  • Maintains an up-to-date active list of all LOCs submitted to include date submitted, decision, follow up, etc.
  • Follows up on all deferred Prospective Enrollees as needed and resubmits required information to DHCS to request approval.
  • Participates in weekly enrollment team meetings as well as IDT and meetings as needed.
  • Assists the transitioning of care and monitoring hospitalizations and SNF patients to ensure appropriate transition.
  • Communicates and interacts with patient caregivers and/or family members as needed.
  • Follows established departmental and position specific policies and procedures and updates as needed.
  • Keep abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government/regulatory agencies.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
  • Attends meetings as required and participates on committees as directed.
  • Performs other duties as assigned or requested.
