About The Position

Under the direct supervision of the Public Health Nursing Supervisor the P4 Program Team Lead will manage, coordinate, and implement grant funded initiatives under the Produce Prescription Pilot Program (P4) awarded to Navajo Health Foundation/Sage Memorial Hospital. The P4 Program Team Lead will display positive teamwork qualities and collaboration by being directly involved in assisting the P4 team members with daily workload and will provide backup and support for the workload. In addition to the P4 program, the P4 Program Team Lead supports the design and implementation of community health initiatives that enhance wellness and access to care. Program areas may include wellness, diabetes, obesity, nutrition, physical activity promotion, gardening, and other community health prevention programs. This list of duties and responsibilities is illustrative only of the tasks performed by this position and is not all-inclusive.

Requirements

  • Bachelor’s degree in public health, health education, community health, human services, communications, or a closely related field, AND at least two (2) years of experience in coordinating community health programs and/or grant funded activities or programs.
  • Must be able to successfully pass the Employee Health Program requirements and background investigation.
  • Knowledge of health issues impacting Navajo Nation communities and interventions used to prevent those health issues.
  • Knowledge of principles and practices of health education, social and behavioral sciences, and health behavior change as they apply to public health; mental health, or human services; or physical and biological sciences as they apply to the environment.
  • Knowledge of principles, methods, techniques, theories and practices of health education and public health or human services.
  • Knowledge of principles and practices of program management, including funding, developing, and evaluating program quality and effectiveness.
  • Ability to demonstrate effective written and verbal communication skills.
  • Ability to establish and maintain effective working relationships.
  • Ability to provide leadership to community coalitions or collaboratives.
  • Ability to operate a personal computer and use common software such as Microsoft suite software and various social media platforms and the demographics that use them..
  • Ability to write or edit clear and effective reports, correspondence, and educational materials.
  • Ability to work independently, meet deadlines and work under pressure.
  • Ability to interpret federal, state, local laws and regulations and applicable data relating to specific health issues (e.g., tobacco, injury prevention) or social service programs (e.g., protective services, cash aid assistance).
  • Ability to work effectively in a team environment, receive constructive feedback from leadership and follow the chain of command.
  • Ability to design or review brochures, advertisements, and newsletters.
  • Ability to evaluate effectiveness of programs, create an evaluation report and submit to Public Health Nursing Supervisor for feedback
  • Experience in utilizing statistical concepts and methods for collecting, compiling, and interpreting data.
  • Experience in creating and conducting effective outreach and utilizing community engagement strategies.
  • Experience in successfully educating youth and adults through presentations, written materials, and media and social media messaging.
  • Experience in program planning, implementation, and assessment.
  • Experience in policy, procedure, and protocol development and revision.

Responsibilities

  • Coordinate and assist team members with deliveries of F&V boxes and gift cards to program recipients.
  • Provide office coverage to be available for participants to contact either in person or through a dedicated P4 main number (cell phone or land line)
  • Work with participants to resolve any issues with vouchers or cards
  • Work with community partners to report and resolve any issues with vouchers or cards.
  • Submit monthly reports (or as needed) to the Public Health Nursing Supervisor on the grant and program activity status. Reports should be a compilation of data with analysis.
  • Work with the P4 Team and Public Health Nursing Supervisor to plan, coordinate, and implement grant-related program activities according to an outlined strategic plan and program objectives
  • Discuss with Community Health Division members the community needs assessment data to identify gaps in services and so changes to strategic plan can be made based on additional needs.
  • Work closely with different SMH departments to coordinate P4 program activities and capitalize on existing resources to carry out grant activities.
  • Write formal letters, meeting agendas, grant summary/data reports, and effectively communicate with team members and Community Health Leadership team.
  • Work with P4 Team to build and foster community partnerships (organizations, businesses, local farmers, etc.) and engage them in policy/procedure/protocol discussions, program development and other programmatic activities to meet grant objectives.
  • In collaboration with the Public Health Nursing Supervisor, work together with partners on local, tribal, state, and national prevention efforts and participate in grant cohort discussions.
  • Assist Public Health Nursing Supervisor with Memorandum of Agreements, sub-award agreements and/or contracts with community partners or vendors as needed.
  • Assist in coordinating services (quotes, payment, schedule) with outside vendors, contractors, etc., to fulfill program activities and practice responsible grant expending.
  • Work with Public Health Nursing Supervisor and P4 Team to identify potential opportunities for training and professional development for all program team members.
  • Assist in the development of a participant packet which includes commitment agreements outlining the requirements of the P4 Program; track compliance and resolve participant noncompliance issues including suspension and/or termination from the program.
  • Assist with designing flyers, brochures, and/or posters to increase awareness of public health issues and promote community events and workshop opportunities, as needed.
  • Prepare and conduct and assigned presentations (in-person and virtual) to stakeholders including but not limited to SMH staff, local government, businesses, and professional groups.
  • Work with the Public Health Nursing Supervisor to develop and implement evidence-based social marketing, awareness campaigns, and public messaging (PSAs), as needed, to increase community awareness on the benefits of F&V and access to programs for youth, adults, and families.
  • Assist with the data collection and reporting requirements
  • Work with the Public Health Nursing Supervisor and P4 Team to evaluate program effectiveness, conduct research, report results to stakeholders, and make recommendations to supervisor to improve programs.
  • Assist with Planning, organizing, implementing, and/or participating in special events, community meetings, focus groups, workshops, in-service trainings, conferences, and funder meetings, as they relate to objectives and goals of the P4 grant and community health.
  • Adhere to SMH operating and financial policies and procedures.
  • Submit requisitions for expenditures within approved budget limit and ensure purchase completion according to Finance policy and procedures, as needed.
  • Participate in discussions and assist with program planning for future potential grant funding programs or succeeding grant terms.
  • Adhere to the Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards for all federal grant programs and other funder requirements.
  • Assist in the preparation and submission of required post-award, programmatic and financial reports to funders in a timely manner.
  • Other duties to accomplish the goals and objectives of the P4 Program and Community Health Division
  • Assist with other Community Health programs as needed.
