About The Position

The Overnight Certified Peer Support Specialist will support recovery and participation by sharing experience with and educating about recovery and rehabilitation, coordinating with internal and external resources, and walking with the person on the path of recovery and community participation, independence, and productivity. Provides street outreach to engage clients who are defined as unsheltered. Provides support services until participant is successfully housed. Refer participants to Drop-in Shelters or the Coordinated Assessment Model for housing placement. Documents services provided in a timely and accurate manner. Perform related duties as assigned by supervisor. Maintain compliance with all company policies and procedures and applicable rules and regulations of grantors. Models the Pillars and guiding principles of NSO at all times.

Requirements

  • High School Diploma or GED.
  • A Peer Support Specialist must have experience using services in community mental health (CMH) for a minimum of one year.
  • Certified Peer Support Specialist
  • Experience with recovery from the impact of a mental illness, addiction, and/or homelessness preferred.
  • At least 6 months experience in the provision of peer support, self-help, or mental health/behavioral health services.
  • Basic computer skills (email, internet use, knowledge of Microsoft Word).
  • Ability to effectively communicate and engage people in the community/consumers.
  • Will drive NSO Vehicle
  • Must have access to reliable private transportation
  • Must maintain valid MI driver’s license/State ID
  • Must be able to visit locations while working for NSO
  • Will be expected to travel on NSO time
  • Must maintain automotive insurance

Responsibilities

  • This position promotes the mission, vision, values and strategic plan of Neighborhood Service Organization and helps to achieve the strategic goals and objectives of the unit.
  • The Overnight Certified Peer Support Specialist will support recovery and participation by sharing experience with and educating about recovery and rehabilitation, coordinating with internal and external resources, and walking with the person on the path of recovery and community participation, independence, and productivity.
  • As part of the Detroit Healthy Housing Center Homeless Recovery Services team and in collaboration with the person served, will provide peer coaching while being responsible for providing basic needs, transportation to drop-in centers, and diversion support.
  • This position is a community-based position, which involves outreach services. The Overnight Certified Peer Support Specialist provides services to clients experiencing unsheltered homelessness, including families with minor children.
  • Gain trust and respect of clients and potential clients based on shared experience, mutual support, and perspectives.
  • Model recovery and skills that are conducive to community participation and inclusion.
  • Provides street outreach to engage consumers who are defined as unsheltered.
  • Provide follow-up services to known locations as identified by the person served.
  • Performs thorough intake and assessments on participants
  • Discusses diversion and explores possible solutions with clients.
  • Mentor the client on negotiating human service systems.
  • Refer participants to the Coordinated Assessment Model for housing placement.
  • Assist participants in understanding and navigating the coordinated entry system.
  • Transports clients to drop-in shelter.
  • Documents services provided in a timely and accurate manner.
  • Participates in team meetings and presents information obtained through the screening and assessment process.
  • Completes progress notes (which includes HMIS service transactions and HMIS case notes) according to agency policy.
  • Completes Assessments in a thorough and timely manner.
  • Completes and makes corrections to reports within time frames requested by supervisor.
  • Follows through with assigned tasks.
  • Participates in supervisory meetings and team meetings.
  • Identifies resources within the community and assists clients to access resources.
  • Develops and maintains a working knowledge and relationship with providers of resources for clients.
  • Apprises administrative staff of service gaps that affect clients’ functioning within the community.
  • Productivity in the Overnight Program will be based on outcome measures predetermined by the funding source and through project assessment.
  • Service Activity Logs must demonstrate performance consistent with stated outcome measure.
  • Productivity of direct service is proportionate to the time dedicated to service provision.
