Requirements

  • Minimum 18 years of age required.
  • High School Diploma or General Education Degree (GED).
  • Six months of related work experience preferred, but not required.
  • Colorado Identification Card (required)
  • Valid QMAP certification required (for QMAP Care Specialist role).

Nice To Haves

  • Six months of related work experience preferred.
  • Qualified Medication Administration Personnel (QMAP) Certification (preferred).
  • CPR Certificate/First Aid Certificate (preferred).
  • De-Escalation Training (preferred).

Responsibilities

  • Demonstrate ethical and professional behavior aligned with Hilltop’s Assisted living Communities (ALCs) values, including Service 1 (how we treat each other) and Service 2 (how we treat our residents), by setting boundaries, being predictable, consistent, kind, and transparent.
  • Administer medications as outlined by physician orders by verifying medications to the Medications Administration Record.
  • Strictly follow the rights of medication administration process including right medication, right resident, right dose, right time, right route, right reason, and right documentation.
  • Promote and participate in positive customer relations by learning and using resident names; acknowledging residents and visitors; answering the phone as needed; having knowledge of the facility, where to direct people, and how to access resources; maintaining confidentiality; and demonstrating a positive attitude.
  • Strictly follow the rights of medication administration process; including right dose, right route, right medication, right client, and right documentation.
  • Follow all Corporate, Program, and State Medication Administration policies, procedures, and regulations.
  • Monitor the narcotic medications by counting all assigned narcotics prior to receiving control over the medication cart; conduct a narcotic count prior to giving control of the medication cart to any other individual.
  • Complete assigned documentation within prescribed timelines promptly, accurately, and legibly.
  • Report all medication errors immediately and completes the required documentation for medication errors.
  • Clean assigned medication carts prior to the end of the shift by emptying trash, wiping down the cart, organizing medications, and replenishing supplies.
  • Answer radio, pager, and the resident request system.
  • Assume Care Specialist duties, as needed.
