About The Position

As an Outreach Worker in the FIT program, you will provide high-quality, community-based services to youth with Serious Emotional Disturbance (SED) and their families. You will work as part of a multidisciplinary team to deliver intensive treatment, support, and skill building to help stabilize youth in their home and community settings.

Requirements

  • Associate’s or Bachelor’s degree in Human Services with relevant experience, or enrolled in a Master’s degree program
  • Bilingual In Spanish (Required)
  • Knowledge of mental illness diagnoses, behavioral modification techniques, and coping strategies
  • Familiarity with child-serving systems and community resources
  • Strong communication, time management, and organizational skills
  • Ability to work with diverse populations and demonstrate cultural competence
  • Valid driver’s license and reliable transportation required

  • Experience providing therapeutic support to youth and families preferred

Responsibilities

  • Deliver intensive in-home and community-based services 3–5 times per week, as clinically indicated
  • Teach and coach youth in coping skills, self-care, and strategies for managing emotions
  • Support families in addressing the emotional and mental health needs of youth
  • Collaborate in the development and implementation of individualized treatment plans
  • Provide behavioral coaching to enhance problem-solving, communication, and family dynamics
  • Facilitate linkages to community resources and make referrals through warm hand-offs
  • Co-facilitate team meetings and assist with aftercare planning
