Our Mission is to provide compassionate care and services to empower individuals experiencing substance use disorders, mental illnesses, and homelessness to attain an enhanced quality of life regardless of ability to pay. The Milestone HOME Team focuses on Engagement (building trust and connection), Basic Needs (providing immediate support), Connection to Care (linking people to services), and Emergency Transportation. This position is responsible for the development and implementation of the Homeless Outreach & Mobile Engagement (HOME) Team Program, including community outreach, networking, transportation, documentation, and advocacy for each client. The role also involves positively representing the agency within the community.
