About The Position

Under the direction of the Wellness Manager, the Outreach Worker works collaboratively as part of the Wellness Team to deliver services tailored to meet client needs. The Outreach Worker provides individual and family-centered, strengths-based, and trauma-informed interventions that support holistic wellness. The Outreach Worker is responsible for providing community-based outreach, including harm reduction services, crisis intervention, education, and system navigation. This role builds trusting relationships with community members, supports access to resources, and facilitates psychoeducational and culturally relevant programming. The Outreach Worker will be familiar with community services and resources and work closely with internal and external partners to ensure coordinated care.

Requirements

  • Minimum of a diploma in Social Work, Psychology, Mental Health, Human Services, or a related field (or an equivalent combination of education and experience)
  • Minimum of one (1) year of experience in mental health and/or wellness, or completion of a relevant certificate program
  • Experience in outreach, harm reduction, and/or mental health services
  • Criminal Record Check with Vulnerable Sector Search with results acceptable to WFN
  • Must be able to lift up to 50 pounds
  • Maintains a high level of confidentiality and uses discretion on all matters relating to the affairs of the Westbank First Nation
  • First Aid and CPR Certification
  • Ability to work flexible hours, including evenings and weekends

Nice To Haves

  • Experience working in a First Nations community setting is an asset
  • Mental Health First Aid and Crisis Intervention Training preferred

Responsibilities

  • Provides outreach, education, prevention, and intervention services within the community
  • Distributes harm reduction supplies and provides education on safer use practices
  • Administers and provides training on Naloxone use
  • Responds to crisis situations and provides immediate support and intervention
  • Assists clients in accessing basic needs, including housing, shelter, food, transportation, and healthcare
  • Provides advocacy and system navigation support to individuals and families
  • Facilitates referrals to appropriate internal and external services and treatment programs
  • Participates in Wrap Around System of Care and coordinated care planning
  • Establishes and maintains trusting relationships with clients through regular contact
  • Promotes culturally appropriate programming, including recreational, social, and cultural activities
  • Networks with internal departments and external agencies to support client needs
  • Collaborate with multidisciplinary team members to ensure continuity of care
  • Maintains accurate, confidential client files and documentation
  • Tracks service statistics and contribute to monthly program reports
  • Performs general administrative duties and supports program development as required
  • Ensures compliance with organizational policies, procedures, and confidentiality standards
  • Performs other related duties as required and directed by the Wellness Manager
