About The Position

This role focuses on patient communication and care gap closure. The Outreach Specialist will be responsible for contacting patients via telephone and mail, collaborating with the Quality Improvement Manager to address care gaps, and initiating care orders as needed. The position involves contacting patients based on various reports and tracking appointments to ensure completion of necessary care. Marginal duties include scheduling transportation, assisting with patient needs, and performing other assigned duties.

Requirements

  • High school diploma and or Certification as a Medical Assistant preferred.
  • Licensed Practical Nurse (LPN) and or Certified Nursing Assistant will be considered.
  • None required experience.
  • Excellent telephone etiquette.
  • Computer use including word processing, excel, and Electronic Health Records.
  • Able to operate office equipment including computer, fax machine, copy machine, etc.
  • Able to follow through with assignments responsibly, accurately, and efficiently.
  • Ability to communicate effectively with and relate to a diverse population in a professional and courteous manner.
  • Ability to follow oral and/or written instructions.
  • Able to work independently and as a team member.
  • Ability to work effectively with all members of the administrative team.
  • Able to communicate effectively with contacts outside of the corporation.
  • Full time, flexible and varied hours. Evening hours may be required.
  • Local travel between GLBHC sites may be required on a limited basis. If usage of a personal vehicle is required, reimbursement will be made in accordance with corporate policy.

Nice To Haves

  • Minimum of one year experience in a medical setting preferred.
  • Knowledge of insurance plans helpful.

Responsibilities

  • Communicates appropriately with patients by telephone and generates/sends letters to patients who are unreachable by telephone.
  • Collaborates with the Quality Improvement Manager to close identified gaps in care. Uses standing orders to initiate care orders as needed for patients.
  • Contacts patients needing services based on third party payer reports and other GLBHC’s population analytic reports.
  • Tracks initial and/or follow-up appointments for completion of needed care. Follows up with patients missing appointments.
  • Contacts patients that need to be scheduled to access care and encourages their continuity and completion of care. Addresses non-engagement of individuals assigned by health plans.
  • Scheduling transportation and/or reaching out to other ancillary staff to assist patient needs.
  • Performs other duties as assigned.
