About The Position

The Outreach Specialist will be responsible for providing outreach, engagement, and program enrollment to Housing Works’ Youth and Prevention Services Healing Pathways Program. The Outreach Specialist will be working out of the East Harlem Community Health Center. This is an in-person role. The Outreach Specialist conducts program prevention services, including targeted outreach, linkage to care, PrEP/PEP, screening, and referrals to various other treatment services to support client care. The Outreach Specialist will assist with the facilitation of Seeking Safety, Anger Management, and other identified program EBIs. The Outreach Specialist conducts field work such as client home visits as needed, linkages in with the community, and participates in community-based and event-based outreach.

Requirements

  • High School Diploma or GED with relevant work experience required

Responsibilities

  • Screen, recruit, and enroll individuals in the program.
  • Refer clients to PrEP Services
  • Participation in community events, such as health fairs, LGBT events, testing events, etc.
  • Provides presentations for internal and external programs to promote the program services.
  • Assist with the recruitment for Evidence-Based Intervention groups
  • Assist with the planning and implementation of events to increase client engagement
  • Conducts field work as necessary to foster client engagement and connection to care
  • Participate in weekly supervisory sessions with the Program Coordinator and Assistant Program Director.
  • Complete documentation, including client assessments, consent forms, and update program trackers.
  • Participate in weekly staff meetings and other departmental and agency-wide meetings as required.
  • Conduct all programmatic outreach activities (e.g., street outreach, shelters, piers, day centers, parks, health fairs, soup kitchens, house balls, and other venues where youth are known to gather).
  • Conducts HIV and Hep C rapid testing.
  • Perform other duties as assigned.
  • Participate in Housing Works advocacy events (minimum 3 in-person events, 4 alternative format events per year, July-June)
  • Participate in monthly coalition meetings, committee meetings, community meetings, and assigned task force meetings.
  • Participate in recurring interagency meetings, as well as external community meetings, to improve integrated care and maintain relationships with community partners
  • Participate in department activities such as new staff onboarding, YPS Pride Committee, CQI efforts, etc
