About The Position
The Outreach Specialist conducts direct community outreach to identify, engage, and assess unhoused veterans across assigned urban and rural territories. This role connects veterans to housing stability and support resources through St. Patrick Center's SSVF program and partner community agencies.
Requirements
- BSW
- Valid driver’s license (Class E for MO residents), active auto insurance, and a reliable personal vehicle required.
- Knowledge of community resources, mental health and substance abuse terminology, and crisis intervention techniques.
Nice To Haves
- Prior experience working with unhoused populations preferred.
Responsibilities
- Conduct field outreach to locate unhoused individuals and complete initial program assessments on-site.
- Engage clients in conversations regarding mental health, substance use, and barriers to housing stability.
- Provide client transportation to essential services and appointments as needed.
- Build relationships with local social service agencies and attend Continuum of Care meetings to promote program resources and referral criteria.
- Forward completed client assessments to case management staff and maintain accurate documentation.
