About The Position

The Outreach Nurse supports the efficient delivery of community-based vaccination and STI services through outreach events. This role works closely our Community Health and Wellness partners and with the Infection Control Nurse and exercises independent judgment in monitoring outreach activities and associated clinical functions. The Outreach Nurse actively participates in quality improvement initiatives, program monitoring, and ongoing education to stay current with advancements in vaccines and STls. Additionally, the Outreach Nurse ensures patient safety during outreach events and plays a key role in expanding NATIVE HEALTH's mobile, school-based, and community-based vaccination and STI services. This work primarily serves underserved and minority populations throughout Maricopa County. This position is dedicated to outreach-based vaccination and STI services under the Shield for Life program and reports directly to the Health Services Director.

Requirements

  • Graduation from accredited school or college of nursing is required.
  • Current licensure to practice nursing in the State of Arizona as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) is required.
  • Minimum of three (3) years of nursing experience is required.
  • Ability to work a flexible schedule, including weekends and evening hours, as needed.
  • Ability to travel throughout Maricopa County to various community-based locations.
  • Ability to stand for extended periods and lift and carry supplies as required.
  • Ability to work outdoors and in non-traditional healthcare settings.
  • Demonstrates dependability, punctuality, reliability, and the ability to work effectively with others while meeting deadlines.
  • Maintains a professional demeanor in behavior, appearance, and all organizational materials produced.
  • Possesses strong critical thinking, problem-solving, and customer service skills with a high level of professionalism.
  • Proficient in computer use, including Microsoft Office applications (Word, Excel, Access, PowerPoint) and internet navigation.
  • Must possess a valid and unrestricted Arizona driver's license (or obtain one within 30 days of hire if relocating from out of state).
  • Must have no DUI/DWI or reckless driving convictions within the past five (5) years and no more than two at-fault accidents within the past three (3) years.
  • Must remain insurable under NATIVE HEALTH's liability auto insurance policy.
  • Must successfully pass a criminal background check and obtain a Class I Fingerprint Clearance Card within ninety (90) days of employment.
  • Must pass pre-employment drug and alcohol screening and comply with random testing during employment.
  • Must provide up-to-date immunization records.
  • Must provide documentation of a current tuberculosis (TB) screening (within the past 12 months) upon hire and maintain annual compliance.
  • Must obtain CPR certification within ninety (90) days of employment and maintain a valid certification throughout employment.

Nice To Haves

  • Knowledge of immunization programs, vaccine storage and handling, STI screening workflows, and state reporting requirements is preferred.
  • Demonstrated commitment to improving the health status of minority, underserved, uninsured, and vulnerable populations, including a willingness to serve underinsured communities.
  • Proficiency with computer applications, including Microsoft Office, Adobe, registries/medical databases, and electronic health record systems (EMR).

  • Experience with immunization programs, including vaccine storage and handling, and reporting to state immunization registries.
  • Experience supporting STI, HIV, HCV, or other infectious disease screening, including linkage-to-care coordination.
  • Experience providing nursing services in community-based, mobile, school-based, or outreach settings.
  • Bilingual in English and Spanish preferred; proficiency in additional languages relevant to the service population is highly valued.

Responsibilities

  • Monitors all outreach events and their clinical operations and related functions.
  • Responsible for the immunizations program and outreach-based STI testing program related to outreach events.
  • Provides nursing services to patients following the Arizona State Board of Nursing scope of practice.
  • Support the organization as needed.
