About The Position

The Outreach Nurse supports the safe and efficient delivery of community-based vaccination and sexually transmitted infection (STI) services. Working closely with Community Health and Wellness partners, the nurse coordinates and monitors outreach activities and related clinical functions while exercising independent professional judgment. The Outreach Nurse ensures patient safety at outreach events and helps expand NATIVE HEALTH’s mobile, school-based, and community-based vaccination and STI services for underserved and minority populations throughout Maricopa County. The position also participates in program monitoring and ongoing professional education to remain current on vaccine and STI-related clinical practices and advancements. The Outreach Nurse Reports administratively to the Health Services Director and works under the clinical oversight of the Chief Medical Officer or designated nursing leader.

Requirements

Graduation from accredited school or college of nursing is required.
Current licensure to practice nursing in the State of Arizona as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) is required.
Minimum of three (3) years of nursing experience is required.
Ability to work a flexible schedule, including weekends and evening hours, as needed.
Ability to travel throughout Maricopa County to various community-based locations.
Ability to stand for extended periods and lift and carry supplies as required.
Ability to work outdoors and in non-traditional healthcare settings.
Demonstrates dependability, punctuality, reliability, and the ability to work effectively with others while meeting deadlines.
Maintains a professional demeanor in behavior, appearance, and all organizational materials produced.
Possesses strong critical thinking, problem-solving, and customer service skills with a high level of professionalism.
Proficient in computer use, including Microsoft Office applications (Word, Excel, Access, PowerPoint) and internet navigation.
Must possess a valid and unrestricted Arizona driver's license (or obtain one within 30 days of hire if relocating from out of state).
Must have no DUI/DWI or reckless driving convictions within the past five (5) years and no more than two at-fault accidents within the past three (3) years.
Must remain insurable under NATIVE HEALTH's liability auto insurance policy.
Must successfully pass a criminal background check and obtain a Class I Fingerprint Clearance Card within ninety (90) days of employment.
Must pass pre-employment drug and alcohol screening and comply with random testing during employment.
Must provide updated immunization (IZ) record at the time of employment.
Must have a current (within 12 months) tuberculosis (TB) skin report upon employment and provide an annual update.
Must obtain Basic Life Support (BLS) certification within ninety (90) days of employment and maintain a valid certification throughout employment.

Nice To Haves

Knowledge of immunization programs, vaccine storage and handling, STI screening workflows, and state reporting requirements is preferred.
Experience with immunization programs, including vaccine storage and handling, and reporting to state immunization registries.
Experience supporting STI, HIV, HCV, or other infectious disease screening, including linkage-to-care coordination.
Experience providing nursing services in community-based, mobile, school-based, or outreach settings.
Bilingual in English and Spanish preferred; proficiency in additional languages relevant to the service population is highly valued.

Responsibilities

Monitors all of the clinical operations and related functions of outreach events.
Coordinates the immunization and outreach-based STI testing programs associated with community outreach events.
Provides nursing services in accordance with the Arizona State Board of Nursing scope-of-practice requirements.
Assists with outreach-program needs and other community-based clinical activities as required.
Completes NATIVE HEALTH’s requirement of eight hours of outreach activity unrelated to the employee’s primary position responsibilities.
Attends and actively participates in required in-services, conferences, training sessions, and division or staff meetings.
Perform other duties as assigned by the supervisor.
