About The Position

The Outreach Navigator/Assessor is responsible for identifying, assessing, and connecting individuals experiencing homelessness with appropriate housing resources through the Coordinated Entry System (CES). This role conducts outreach, completes housing assessments through the Homeless Management Information System (HMIS), and provides pre-navigation services to assist individuals in obtaining required documentation for housing programs such as Permanent Supportive Housing (PSH). This role is accountable for delivering measurable results aligned with departmental and organizational goals and ensuring alignment with our company vision and core values.

Requirements

  • Minimum two (2) years of experience in social services, housing programs, homelessness outreach, or case management.
  • Experience working with vulnerable populations including individuals experiencing homelessness.
  • Strong ability in client engagement, outreach, and housing navigation.
  • Demonstrated competency in data entry and documentation within HMIS or similar case management systems.
  • Strong communication, problem-solving, and interpersonal skills.
  • Ability to manage multiple priorities while maintaining accuracy and timeliness in documentation.
  • Ability to work both independently and collaboratively within a multidisciplinary team.
  • Proficiency with Microsoft Office Suite (Word, Excel, Outlook).

Nice To Haves

  • Bachelor’s degree in social work, Human Services, Public Health, or a related field equivalent experience.
  • Valid driver’s license is preferred (for outreach and transportation duties).
  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Conduct outreach and complete client enrollments and housing assessments for individuals experiencing homelessness through the Coordinated Entry System (CES).
  • Provide pre-navigation services to assist individuals in obtaining eligibility documentation required for Permanent Supportive Housing (PSH) and Rapid Rehousing (RRH).
  • Dedicate a minimum of four (4) hours per week to community outreach activities, including community calls, in-reach efforts, and encampment reconnaissance.
  • Provide transportation and logistical support to assist clients with housing-related activities including apartment move-ins, obtaining identification documents, Social Security cards, disability verification, homeless verification documentation, and SSI award letters.
  • Attend required Housing Assessor, Housing Navigator, and HMIS trainings to effectively utilize the Coordinated Entry System.
  • Participate in training required by the funding source and Avenue 360.
  • Maintain proficiency in HMIS, Coordinated Entry System procedures, and housing navigation protocols.
