About The Position

Sonoran Prevention Works is a statewide harm reduction nonprofit working to build a healthy Arizona with people who use drugs. SPW takes a systems-based and person-first approach to drug use, working to support individuals in achieving safety and health, provide technical support to organizations who interface with people who use drugs, and change local and state-level policy to embrace evidence-based changes. SPW expects all its staff to engage on a journey of understanding and working toward racial and social justice. The organization currently maintains a staff of approximately 40 employees across Arizona, with a budget exceeding $4 million. Reporting to an Outreach & Navigation Manager, the Outreach & Navigation Specialist is a cross-trained harm reduction professional responsible for delivering integrated syringe services, testing, resources, and systems navigation to people who use drugs across Arizona. This position ensures that participants have equitable access to safer use supplies, testing, overdose prevention tools, education, and referrals to supportive services. The Outreach & Navigation Specialist provides direct participant support in field, mobile, and community settings while maintaining accurate data and fostering relationships with partner agencies. This role blends participant engagement, health education, and navigation in alignment with Sonoran Prevention Works’ mission and harm reduction principles. This position will complete regular fixed responsibilities, as well as have the opportunity to take on occasional special projects.

Responsibilities

  • Distribute safer use and overdose prevention supplies via fixed site, mobile, and street-based outreach.
  • Conduct HIV and Hepatitis C rapid tests and provide post-test counseling, education, and linkage to care.
  • Conduct blood draws (phlebotomy) for HIV and HCV reactive participants once trained and training provided by SPW (as applicable).
  • Deliver individualized harm reduction education focused on safer use, wound care, and overdose prevention.
  • Build trusting relationships with participants and ensure low-barrier, judgment-free access to services.
  • Identify new outreach locations and populations for engagement and harm reduction delivery.
  • Support community and partner events providing harm reduction education and materials.
  • Provide warm handoffs and follow-up support for participants seeking treatment, healthcare, or other resources.
  • Maintain updated knowledge of local housing, healthcare, legal aid, and social service resources.
  • Serve as a liaison between participants and community organizations to reduce systemic barriers.
  • Advocate for participant needs through trauma-informed and person-centered navigation.
  • Maintain timely and accurate data entry in accordance with program and funder requirements.
  • Assist with inventory management and kit-making activities.
  • Participate in all required check-ins, team meetings, and trainings.
  • Support quality assurance and data integrity processes for program reporting.
  • Contribute to department-wide initiatives and special projects as assigned.
  • Other duties as assigned by management in the employee’s line of reporting.
  • Attend relevant scheduled meetings.
