About The Position

Cook Hospital and Care Center is seeking an experienced healthcare professional to serve as our Outreach Manager. This position is responsible for the coordination, development, and day-to-day oversight of the hospital’s outpatient and outreach services, helping ensure accessible, high-quality services for patients throughout our community and surrounding region. The Outreach Manager oversees a diverse portfolio of outpatient services, including Telemedicine, Specialty Clinics, Outpatient IV Infusions, Outpatient Procedures, Cardiac Rehabilitation, Pulmonary Rehabilitation, and other outreach services. This position works closely with physicians, providers, clinical departments, staff, and external healthcare partners to coordinate services, develop programs, improve patient access, and support continued growth of outpatient care. The Outreach Manager also serves as the hospital’s Trauma/Stroke Coordinator, supporting program coordination, quality improvement, regulatory requirements, education, reporting, and collaboration with regional partners.

Requirements

• Bachelor's degree in healthcare administration, business, marketing, nursing, public health, or a related field preferred.
• Healthcare experience with demonstrated knowledge of community outreach, referral development, care coordination, or program management.
• Experience working with healthcare providers, community organizations, and referral partners.
• Strong communication, relationship-building, organizational, and presentation skills.
• Ability to work independently while collaborating effectively with physicians, clinical staff, leadership, and community partners.
• Experience with trauma, stroke, cardiac rehabilitation, pulmonary rehabilitation, or healthcare quality improvement is preferred.
• Strong computer, documentation, and data-tracking skills.
• Valid driver's license and reliable transportation for community outreach activities.

Responsibilities

• Oversee and coordinate assigned outpatient and outreach services, ensuring effective day-to-day operations and a positive patient experience.
• Coordinate Telemedicine services, including scheduling, equipment, provider coordination, staff training, and related operational needs.
• Coordinate and support Specialty Clinics, working with visiting specialists, providers, staff, and patients to ensure efficient clinic operations.
• Oversee Outpatient IV Infusion and Outpatient Procedure services, including coordination of scheduling, staffing, supplies, equipment, and patient care processes.
• Oversee and support the development and operations of Cardiac and Pulmonary Rehabilitation services.
• Identify opportunities to expand outpatient services and improve access to specialty and regional healthcare services.
• Collaborate with physicians, providers, clinical departments, and external healthcare organizations to establish and maintain effective service relationships.
• Monitor program performance, patient volumes, utilization, and outcomes and identify opportunities for improvement.
• Participate in budgeting, purchasing, equipment planning, and resource management for assigned services.
• Develop and maintain policies, procedures, workflows, and processes necessary for effective program operations.
• Ensure services are delivered in accordance with applicable regulatory, accreditation, and organizational requirements.
• Coordinate the overall operation and development of Cardiac and Pulmonary Rehabilitation services.
• Coordinate patient referrals, evaluations, treatment plans, exercise programs, monitoring, education, and follow-up.
• Monitor patients during rehabilitation activities, including appropriate assessment of vital signs, heart rate, blood pressure, respiratory status, and EKG monitoring as applicable.
• Collaborate with physicians and other healthcare professionals regarding patient care and program needs.
• Maintain appropriate patient records and documentation.
• Promote the programs to providers, patients, and the community to support appropriate referrals and participation.
• Participate in quality improvement, program evaluation, budgeting, equipment needs, and continued program development.
• Serve as the hospital’s Trauma/Stroke Coordinator and coordinate activities necessary to maintain applicable trauma and stroke program requirements and designations.
• Monitor program compliance, quality measures, reporting requirements, and performance improvement activities.
• Coordinate trauma and stroke-related education and resources for hospital staff.
• Collaborate with physicians, nursing staff, EMS, regional healthcare organizations, and other partners to support effective trauma and stroke care.
• Participate in trauma and stroke case reviews, quality improvement activities, site visits, and related program evaluations.
• Maintain required documentation and reports and assist with identifying opportunities to improve patient outcomes and processes.
• Support education and community awareness related to stroke and trauma prevention and treatment.
• Promote Cook Hospital & Care Center’s outpatient and specialty services throughout the community and surrounding region.
• Develop relationships with physicians, clinics, healthcare organizations, community agencies, and other referral partners.
• Represent the hospital at community events, health fairs, educational programs, and other outreach activities.
• Identify community healthcare needs and opportunities for new or expanded outpatient and outreach services.
• Develop and implement strategies to increase awareness, appropriate referrals, and utilization of hospital services.
• Collaborate with Marketing and hospital leadership on outreach, communication, and promotional efforts.
