The Outreach Coordinator is responsible for coordinating services for new families referred by the Regional Centers. This role serves as the first point of contact for families starting services, ensuring timely communication, accurate documentation, and seamless coordination with Regional Center staff. The Outreach Coordinator tracks referrals, ensures provider applications and agency care plans are complete, and collaborates with Community Partnerships and Operations teams to deliver a high-quality experience. This is a hybrid position, coming into the Fullerton office 1x per week. Please note that this is a temporary position.
