About The Position

VillageCareMAX is seeking a motivated individual for a Full-Time Outreach Coordinator role. The primary focus of this position is to assist members in coordinating services, specifically ensuring they complete preventive care exams and helping to schedule these appointments. Outreach Coordinators are responsible for all outbound communications to members via phone calls and mail. VillageCare offers a broad spectrum of at-home and community-based services, alongside managed long-term care options, aiming to align with individual needs to foster and sustain the highest possible level of independent living. Requirements

3+ years job related experience.
Excellent communication skills.
Organizational skills.
Problem solving skills.
Bilingual Required.
Must be able to travel to office and monthly events.
Must reside within the New York Tri-State Area - NY, NJ, or CT.

Responsibilities

Ensure that our community of members complete their preventive care exams and assist in scheduling those exams.
Handle all outbound communications to our members via phone calls and mail correspondence.
Handle all incoming telephone calls and answering questions for members and providers.
Assist members in accessing all necessary covered and non-covered services in conjunction with plan of care.
Contact members and engage in QA conversations/questions.
Inquire if members had preventive care exams and confirm appointments.
Assist members in arranging appointments with physicians and other health care providers.
Coordinate pick up /drop off transportation to and from scheduled appointments.
