About The Position

Rooted in aloha and kuleana, Hale Kipa is dedicated to supporting and empowering Hawai‘i’s youth, young adults, and families who are navigating trauma, injustice, and systemic barriers. Through safe and nurturing housing, culturally grounded care, and trauma-informed relationships, we encourage them to discover their passions, embrace their resilience, and thrive as compassionate, responsible leaders in their families and communities. Since 1970, Hale Kipa has championed Hawai'i's youth and children. We provide youth outreach, independent living facilities, therapeutic foster care, a haven for runaways, and more. Our founders recognized a need for a nonprofit to step in to assist this deserving population and we are still doing that critical work today, on O'ahu, Kaua'i, and the Island of Hawai'i. The Outreach Case Worker provides comprehensive, client-centered, and goal-oriented services to at-risk and homeless youth and young adults. This role focuses on empowering clients toward maximum self-determination by assessing individual needs, developing appropriate service plans, and connecting them to essential resources and support systems. Through consistent outreach efforts, including street-based engagement, drop-in center presence, and fieldwork conducted during nontraditional and flexible hours, the Outreach Case Worker identifies and connects with vulnerable youth where they are. A core responsibility of this role is building trusting relationships with youth and young adults through ongoing engagement, meeting immediate needs such as food, clothing, and harm-reduction supplies, and providing information, referrals, and service options. The Outreach Case Worker conducts intakes and assessments, develops developmentally appropriate service plans, and supports clients through crisis intervention, ongoing case management, discharge planning, and aftercare services. Working collaboratively within a multidisciplinary team, the Outreach Case Worker coordinates care across community partners, social service agencies, and other stakeholders to ensure a unified and effective service strategy. This includes maintaining consistent communication with all parties involved in a client’s care and facilitating access to a broad network of community resources. In addition, the Outreach Case Worker represents Hale Kipa with professionalism and integrity in all interactions, maintaining strong partnerships with community organizations and service providers. The position requires adherence to organizational policies, ethical standards, and confidentiality regulations, including HIPAA, while completing all required documentation in a timely and professional manner. Overall, this role plays a vital part in supporting youth experiencing instability by providing accessible, responsive, and coordinated services that promote safety, resilience, and long-term well-being.

Requirements

  • A Bachelor's degree in Social Work, Psychology, or related field preferred.
  • At least one year of experience working with youth/young adults is essential and preferred.
  • Exceptional verbal and written communication skills.
  • Proficiency in navigating electronic health record systems.
  • Consistent and timely documentation.
  • A vehicle for the purpose of transporting youth.
  • A valid driver's license.
  • Current auto insurance and safety check.
