About The Position

Catholic Charities of Broome County is seeking an Outreach Case Manager for its Youth, Family & Community Services division. This role focuses on providing outreach and case management services to individuals and families facing food insecurity, assisting them in obtaining entitlements and connecting them with necessary resources. The position involves direct client interaction, application assistance, needs assessments, and community engagement.

Requirements

  • Good oral and written communication skills.
  • Demonstrated leadership ability and ability to work independently.
  • Excellent knowledge of community resources for all populations.
  • Demonstrated ability to establish rapport and relationships with clients and area agencies.
  • Good interpersonal skills.
  • Computer skills adequate to job duties; familiarity with electronic records and data collection.
  • Knowledge of Broome County area.
  • Valid NYS or PA driver’s license and ability to drive agency vehicle
  • Must be able to lift over 40 lbs.

Responsibilities

  • Participate in staff meetings and agency trainings.
  • Participate in monthly supervision with program supervisor.
  • Responsible for case record documentation and daily and monthly record keeping.
  • Represent the Agency and Program in a positive manner.
  • Provide outreach services to individuals in the community in need of assistance obtaining entitlements to reduce food insecurity.
  • Complete applications for assistance through public service programs/DSS with clients.
  • Link clients to necessary services and act as an advocate.
  • Complete individual needs assessments.
  • Develop a person-centered plan with the client to obtain entitlements.
  • Provide education and information to clients in need of nutritional supplement services.
  • Work closely with local government offices to ensure that client applications are processed correctly.
  • Provide clients with referrals to other nutrition assistance programs and community resources.
  • Coordinate with other service providers and attend meetings as necessary.
  • Attend community events to promote the program and represent the agency.
  • Coordinate with supervisor and team members outreach activities on a weekly basis.
  • Travel throughout Broome County to provide services in urban and rural communities.
