About The Position

The Colorado Coalition for the Homeless seeks an Outreach Behavioral Health Clinician II who works directly with street outreach workers to identify and engage individuals living on the streets and other places not meant for human habitation who are in need of behavioral health care, but for a variety of reasons are unable to access care on their own. The Outreach Behavioral Health Clinician II works directly with other care providers to provide assessments and related diagnoses, consultation, education, brief therapeutic interventions, and referral/linkage for individuals with behavioral health issues. The Outreach Behavioral Health Clinician assists clients in navigating community based behavioral health and health care systems for optimal care. This position is located at 2100 Broadway Denver, CO 80205 and reports to the Clinical Program Manager. NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Requirements

  • Master’s degree in social work, counseling or similar field.
  • Licensed as a LCSW or LPC preferred, with a CAC II or higher strongly preferred. If not licensed, must already be registered with DORA as a candidate for licensure (i.e. LSW,LPCC) and within 6 months of obtaining LCSW or LPC license.
  • Experience interacting with patients in integrated health care, mental health care, or substance use treatment settings preferred.
  • Valid driver’s license required. This requirement may be waived, if necessary, based on overall candidate experience and current needs of the business.

Responsibilities

  • Performs intensive outreach and engagement services to invite adults to engage with recovery supports through referrals to behavioral healthcare, primary care, case management, peer support, housing, and related services.
  • Provides targeted brief interventions and therapy, which is often crisis and recovery focused and episodic in nature.
  • Provides individual and group therapy for adults who are interested in ongoing behavioral health care, using treatment models including motivational interviewing, solution focused therapy, harm reduction, and cognitive behavioral therapy.
  • Completes biopsychosocial assessments and utilizes DSM-V-TR criteria for diagnosis.
  • Develops comprehensive treatment plans, monitoring patient progress and accessing direct psychiatry and medical care through Stout Street Health Center
  • Collaborates with the consulting psychiatric prescribers and primary care physicians as part of the integrated health team.
  • Provides proactive collaborative services to clients/patients with face-to-face and telephone contacts, or as otherwise appropriate.
  • Serves as liaison with collaborating agencies to establish and maintain effective working relationships with organizations and groups providing services to shared clients (care coordination).
  • Performs the initial intake in electronic health record (EHR) for clients/patients who want to connect with psychiatric care at Stout Street Health Center and completes additional paperwork as required.
