About The Position

The Department of Health and Human Services (DHHS) Public Health Nurse II Opening Date: June 12, 2026 Closing Date: June 26, 2026 Job Class Code: 4055 Grade: 07-22 (Nurses) Salary: $64,064.00 - $87,318.40 per year This is inclusive of a 15% recruitment and retention stipend Position Number: 02033-0381 Location: 286 Water Street, Augusta This position allows for partial telework with management approval. Currently, this position is not eligible for visa sponsorship or STEM OPT extensions. Core Responsibilities: The Referral Systems Coordinator applies the nursing process to support individuals and families through health assessments, coordinating referrals, and evaluating outcomes. This role ensures that clients’ healthcare needs are met through collaboration with hospitals, medical providers, schools, and social service agencies. The position also involves outreach to referral sources to provide Public Health Nursing (PHN) updates and education on referral processes, while serving as a key liaison between the department and community partners to promote optimal outcomes for populations served by PHN.

Requirements

  • Bachelor’s degree in nursing -OR- graduation from an accredited school of nursing and 2 years of professional nursing experience.
  • Licensed as a Registered Professional Nurse as issued by the State Board of Nursing.

Responsibilities

  • Apply the nursing process, including assessment, planning, implementation, and evaluation, to address client needs
  • Coordinate care with healthcare providers, schools, and social service agencies to ensure appropriate service delivery
  • Provide outreach to referral sources to educate on PHN, increase prenatal and postpartum referrals, and provide technical support for completing referrals
  • Serve as a liaison between the Department and internal and external partners to promote effective referrals and collaboration
  • Demonstrate strong customer service skills in all interactions with clients, colleagues, and community members
  • Assess community and regional public health needs to inform quality improvement strategies
  • Collaborate with the PHN I Intake Referral Nurse and PHN Office Associates to define roles and streamline referral workflows
  • Participate in PHN workgroups as a representative of PHN outreach and referral processes
  • Work with the PHN I Intake Referral Nurse and PHN Office Associates to develop processes for reporting client referral status to stakeholders
  • Prepare reports, records, and surveys to document outreach activities and referral data
