Crossroads Health is a 501(c)(3) non-profit organization serving Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, and extended housing and recovery services. The Outpatient Therapist position is located at our Youth Residential Campus, New Directions, which has been in operation since 1981. New Directions provides a full continuum of treatment programming for adolescents aged 14-17 struggling with substance abuse disorders and co-occurring mental health concerns. The facility offers gender-specific residential programs, an intensive outpatient program (IOP), and traditional outpatient programs. The Outpatient Therapist will provide comprehensive substance use assessments, develop individualized treatment plans, and facilitate therapeutic services for adolescents. This role supports youth and families through trauma-informed care in an outpatient and IOP setting. Responsibilities include conducting biopsychosocial assessments, risk assessments, and substance use screenings; determining appropriate level of care; coordinating admissions; collecting financial support information; co-facilitating IOP group therapy sessions; facilitating individual and family therapy sessions; and coordinating admissions into other levels of care. This licensed mental health provider will be a member of a collaborative team and requires evening availability.
