About The Position

Join our Grove City Family Medicine team and make a difference in the lives of patients ranging from newborns to geriatrics. This role offers the opportunity to work in a busy, fast-paced environment where no two days are the same. You’ll support patients and providers through a variety of clinical needs while helping to create a positive, welcoming care experience. As an RN, your responsibilities will include patient phone triage and in-basket and managing patient education and follow-up. You’ll serve as a vital link between patients and Providers, ensuring timely and efficient communication through EPIC. Flexibility, attention to detail, and strong organizational skills will help you succeed in this diverse and rewarding role. This position is well-suited for an RN who enjoys variety and thrives in a collaborative environment. You’ll work closely with Physicians, Medical Assistants, and Residents, gaining exposure to a wide range of patient care needs. Provides general nursing care to patients and families throughout the continuum of care in diverse health care settings. The RN is accountable for the practice of nursing as defined by the Ohio Board of Nursing. The RN delegates nursing tasks to licensed practical nurses (LPNs) and unlicensed assistive personnel (UAPs) using the criteria written in the Ohio Nurse Practice Act. According to department policies and nursing care standards, provides professional nursing care services to patients. Responsible for assessing, planning, implementing, and evaluating nursing care provided to assigned patients.

Requirements

  • Associate's Degree (Required)
  • Current Registered Nurse license from the State of Ohio and/or Current Registered Nurse license in good standing with a multi-state compact state.
  • BLS certification
  • BLS - Basic Life Support - American Heart Association
  • RN - Registered Nurse - Ohio Board of Nursing

  • Experience in outpatient or inpatient care is helpful

Responsibilities

  • Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual.
  • Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs.
  • Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data.
  • Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance.
  • As a High Reliability Organization (HRO), responsibilities require focus on safety, quality and efficiency in performing job duties.
  • Patient phone triage and in-basket management.
  • Managing patient education and follow-up.
  • Serving as a vital link between patients and Providers, ensuring timely and efficient communication through EPIC.
