About The Position
A healthcare facility is seeking an Outpatient Physical Therapist to provide patient-centered rehabilitation services within an outpatient setting. This position supports individuals through evaluation, treatment planning, and therapeutic interventions designed to improve mobility, function, and quality of life.
Requirements
- Bachelor's degree in a clinical discipline (e.g., RN, PT, OT, SLP, or SW) or healthcare management required; Master's degree preferred.
- Minimum of 3 years of experience in rehabilitation leadership or administration.
- Valid clinical license as applicable by state.
Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical needs and functional goals.
- Provide outpatient physical therapy services using evidence-based treatment techniques.
- Perform therapeutic exercises, interventions, and education to support patient recovery and independence.
- Monitor patient progress and adjust treatment plans as needed.
- Document evaluations, treatments, progress notes, and other required clinical information accurately.
- Collaborate with physicians, healthcare providers, and rehabilitation team members to coordinate patient care.
- Educate patients and families on treatment plans, home exercise programs, and injury prevention strategies.
- Maintain compliance with clinical standards, safety guidelines, and regulatory requirements.
