The Outpatient Pharmacist works within a closed retail pharmacy located in a free-standing Cancer Center, supporting a specialized patient population including oncology and transplant patients, as well as employees and their families. In this lower-volume, highly focused setting, the pharmacist fills approximately 35 prescriptions per day, including oncology treatments and supportive care medications. Responsibilities include medication dispensing, managing prior authorizations, coordinating copay assistance, and providing chemotherapy education. This role partners closely with providers to ensure safe, effective, and patient-centered care, contributing to positive outcomes in a highly collaborative clinical environment. A caregiver in this position could work any two days during Monday through Friday, from 8:00am to 4:30pm. No holidays or weekends. This role will be stationed at the Sandusky Cancer Center.

Requirements

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations

Nice To Haves

  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area (i.e. Critical Care, Oncology
  • Board Certification
  • Specialty pharmacy experience or background in oncology

Responsibilities

  • Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Contribute to medication safety initiatives.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Be accountable for drug use policy adherence.
