About The Position

Provides pharmacy services, to include: inpatient drug distribution, IV admixture, drug information, pharmacokinetics, drug therapy monitoring, patient education, and controlled substance regulation. Responsible for providing direct supervision of supportive personnel within the Department of Pharmacy. Actively participates in the creation of an environment that fosters patient, family, physician, and staff satisfaction.

Requirements

Bachelor’s degree in Pharmacy
Must be a registered pharmacist, licensed in the state of practice or license eligible.

Responsibilities

Interprets and processes all medication orders. Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques.
Sustains the formulary by minimizing nonformulary procurements, utilizing the therapeutic substitution policy and promoting rational medication therapy selection.
Monitors all new medication orders against the patient’s medication profile for contraindications, drug–drug interactions, drug–food interactions, drug–laboratory test interactions, allergies, and appropriateness of drug, dose route, frequency, and therapeutic duplications. Resolves identified problems with the physician or nurse; and documents these activities as appropriate.
Answers all drug information questions accurately using drug information resources available in the department and documents these activities as appropriate.
Reads, extracts, and interprets information in patient charts accurately.
Provides professional review, quality control checks, and/or direct provision of service for: all unit dose packaging program, all admixtures and sterile products including formulation, stability and sterility consideration, extemporaneous compounds, medication order entry, floor stock orders and controlled substances.
Documents all clinical activities and interventions accurately and completely.
Obtains required continuing education for state licensure.
Provides clinical consultation and clarification to practitioners; recommends evidence-based medication therapy regimens and monitoring plans; and suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Performs clinical services as required including pharmacokinetic dosing, renal dosing, etc.
Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.
Conducts patient education/counseling to inpatients and/or outpatients as requested.
Collaborates with other team members in meeting departmental goals. Establishes and maintains mechanisms for communication and constructive problem solving with staff. Utilizes a team approach in decision-making and problem solving. Builds trust and mutual respect.
Keeps director informed of any unexpected developments that may affect patient care or the budget of the department.
Supervises and directs pharmacy support personnel; verifies the daily activities of pharmacy technicians; and participates in the performance management process as requested. Supervises and instructs students.
