The Outpatient Pharmacist provides comprehensive pharmaceutical care within a high-touch ambulatory retail pharmacy, supporting medication access, prescription fulfillment, and transitions of care for patients across the continuum. Located on the second floor with convenient patient access, this role combines traditional outpatient pharmacy practice with enhanced clinical collaboration, allowing pharmacists to work at the top of their license. Responsibilities include verifying and dispensing prescriptions, performing clinical medication reviews and therapy assessments, counseling patients on safe and effective medication use, and resolving insurance, billing, and medication access issues. The pharmacist partners closely with physicians, nurses, care coordinators, and bedside teams to support discharge planning and continuity of care while supervising pharmacy technicians and ensuring efficient daily operations, regulatory compliance, and the highest standards of patient safety.

Requirements

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations

Nice To Haves

  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
  • Board Certification
  • Licensed Pharmacist with ambulatory/outpatient pharmacy experience

Responsibilities

  • Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Contribute to medication safety initiatives.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Be accountable for drug use policy adherence.
