About The Position

This role is for a full-time Physical Therapist with expertise in orthopedics and sports-related injuries. The therapist will partner closely with Yale Medicine Orthopedics and referring providers to deliver coordinated, high-quality care. The position aims to advance musculoskeletal services and improve patient outcomes through evidence-based practice, return-to-sport rehabilitation, and interdisciplinary collaboration. Requirements

Graduate of a CAPTE accredited program in Physical Therapy or equivalent and B.S. degree
No experience required.
Must maintain a current unencumbered license or temporary permit to practice as a Physical Therapist in CT.
All newly hired employees must have current BLS certification or obtain within two months of hire.
Demonstrate age-specific knowledge in basic anatomy and physiology, psychology and Physical Therapy theory, evaluations, and treatment techniques.
Observation skills
Communication skills (both verbal and written)
Ability to work with patients, families and other providers in a synergistic manner
Willing and able to assume increased responsibilities as required.

Nice To Haves

One (1) to two (2) years' experience in an academic/large medical center based practice preferred.
Experience working with children and/or adults depending on the assigned setting.
Clinical experience with a variety of diagnoses to include post-surgical, developmental, neurological, genetic, oncologic, and traumatic conditions.

Responsibilities

Perform basic clinical evaluations based on physician referrals.
Demonstrate technical skill and accuracy in performing evaluations, treatment planning, and providing therapy, considering age-specific criteria.
Develop population-specific treatment programs, monitor patient responses, and terminate treatments when appropriate.
Set functional, measurable, and appropriate goals based on evaluation findings with patient/family involvement.
Provide comprehensive and appropriate patient and family education.
Participate constructively and collaboratively in team conferences, rounds, clinics, and discharge planning.
Initiate discharge planning and make appropriate recommendations to community resources.
Utilize assistant and support staff and volunteers appropriately and safely in patient care delivery.
Address patient care handoff to ensure continuity of care during scheduled time off.
Function as a collaborative interdisciplinary team member, communicating findings and care plans consistently.
Follow through with requests for changes in treatment plans, re-evaluations, and completion of discharge reports and notes.
Serve as a contact person for family, referral sources, funding sources, and other outside agencies.
Ensure discharge planning and arrangements for necessary follow-up services are in place.
Initiate interdisciplinary referrals when appropriate.
Appropriately delegate responsibility to therapy assistants, clinical students, and support staff, and maintain an appropriate level of clinical supervision and documentation review/co-signature.
Maintain concise and accurate patient care documentation and billing in accordance with policy and regulatory requirements.
Re-evaluate and update patient goals and plans as necessary based on patient response to treatment.
Submit payroll information, reimbursement reports, and day off requests according to standards.
Participate in professional development and stay informed of current developments in the field.
Update assessment and therapy procedures to integrate newly acquired information.
Assist with orientation, onboarding, education, and guidance of therapy staff, support staff, students, and volunteers.
Meet required educational contact hours to maintain Connecticut state licensure and maintain documentation of continuing education.
Use equipment in a safe and effective manner and report malfunctioning equipment.
Maintain a safe and orderly work environment.
Maintain a high standard of professional accountability with respect to safety, efficiency, ethics, and legal issues.
Demonstrate compliance with all hospital and department policies, procedures, standards, and regulations.
Plan and organize daily schedule for optimal time utilization.
Participate in department Quality Assurance activities.
Manage patient caseload to meet productivity standards.
Assume other clinical responsibilities as assigned.
Provide weekend and holiday coverage as per schedule.
Participate consistently in team and departmental meetings and report pertinent information on a timely basis.
