About The Position

BAYADA Home Health Care is seeking an experienced Outpatient Occupational Therapist for a Part-Time or Per Diem opportunity in Baltimore County, Maryland. This role involves servicing seniors within 1-2 designated senior living communities, offering a home health experience with a reduced commute. As a key member of a multi-disciplinary team, you will provide rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining in their homes. Requirements

A current Maryland Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop care plans for patients in senior living communities or group homes for adults with intellectual disabilities.
Operate at the top of licensure using current research-based approaches to maximize client functional independence.
Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment.
Educate clients, families, and community caregivers on care plans to foster a team approach for maximizing functional independence.
Perform evaluations, treatments, and discharge planning using the outpatient EMR platform to document individualized treatment approaches.
