About The Position

The Outpatient Float Pharmacist supports multiple west-side outpatient pharmacy locations including Fairview, Avon, Lutheran, Strongsville, Medina, and occasional coverage at Main Campus on Euclid Avenue. These high-volume outpatient pharmacies are primarily located within Family Health Centers connected to hospital campuses and support a diverse patient population across multiple service lines. This role requires adaptability across varying workflows, teams, and operational environments while maintaining continuity of care and medication accessibility. Responsibilities include floating between designated pharmacy locations based on operational needs, verifying and dispensing prescriptions with a focus on safety and accuracy, conducting clinical review and order validation, managing prior authorization reviews during remote work weeks, providing patient counseling and medication support, and collaborating closely with pharmacists, technicians, and healthcare teams to ensure efficient operations and optimal patient care. A caregiver in this position will work rotating weekday schedules based on site needs, with shifts beginning as early as 7:00am and ending as late as 8:00pm. Schedules are provided approximately two months in advance. This role includes one remote work week every four weeks focused on prior authorizations, a weekend rotation of one weekend every six weeks beginning in 2027 at Avon Family Health Center, and approximately one holiday every five years at the Euclid Avenue/Main Campus location. There are no routine Sunday shift requirements.

Requirements

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Completion of a Board of Pharmacy approved course in the administration of immunizations within 90 days of hire for those administering immunizations

Nice To Haves

  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience
  • Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists hired in a Specialized Pharmacy practice area
  • Board Certification

Responsibilities

  • Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Provide safe, efficient, patient-centered, evidence-based medication services.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Contribute to medication safety initiatives.
  • Document services in a complete, accurate and timely manner.
  • Ensure all pharmacy operations comply with national accreditation standards.
  • Maintain professional interactions with patients to facilitate successful patient experiences.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Be accountable for drug use policy adherence.
